1. What This Page Covers

Blue Rock Physicians Group is a physician-led management services organization. We provide administrative and operational support — credentialing, staffing support, compliance support, practice management, and related technology services — to healthcare organizations. We do not provide medical care, dispense medications, or operate a pharmacy.

This page describes the rights you have in dealing with us as a company: your privacy rights over information you send us through our website, our non-discrimination and accessibility commitments, and how to raise a concern.

This page is not a notice of privacy practices, and it does not describe rights in any course of medical treatment. If you are a patient, the rights you hold over your medical records are exercised with the provider who treats you — see Section 3.

2. Your Privacy Rights

Our website is informational. The only information we hold about you is what you choose to send us by email or tell us by phone — typically your name, contact details, and the content of your message — along with standard server logs. The contact form on our site does not send anything itself: it opens a pre-filled draft in your own email application, which you review and send. Our Privacy Policy describes this in full.

With respect to that information, and subject to applicable law, you have the right to:

• Access. Ask what personal information we hold about you and request a copy of it.
• Correct. Ask us to correct information that is inaccurate or incomplete.
• Delete. Ask us to delete information you have sent us, subject to exceptions such as records we must keep to meet a legal obligation or to establish, exercise, or defend legal claims.
• Portability. Request a copy of your information in a portable, commonly used format, where applicable.
• Opt out of marketing. Tell us to stop sending you promotional messages at any time. We may still reply to an inquiry you sent us.
• Non-discrimination. Exercise any of these rights without being treated differently for having done so.
• Appeal. Ask us to reconsider if we decline a privacy request, where a right of appeal is provided by applicable state law.

We do not sell personal information, share it for cross-context behavioral advertising, use it for profiling, or run analytics or advertising trackers on the website. Residents of California and of other states with comprehensive privacy laws may have additional rights, described in Section 9 of our Privacy Policy.

How to Exercise These Rights

Please tell us what you are asking for and include enough detail for us to locate your information, such as the name and email address you used when you contacted us. We may need to verify your identity before acting on a request, and we will respond within the timeframe required by applicable law.

If There Is a Security Incident

If information you sent us is involved in a security incident, we will notify you and any regulator as required by applicable state and federal law, and we will tell you what happened, what information was involved, and what steps we are taking.

3. If You Are a Patient

Blue Rock Physicians Group does not treat patients and does not maintain patient medical records or prescription histories. Rights over your health information under the Health Insurance Portability and Accountability Act (HIPAA) — including the rights to access and obtain a copy of your records, request amendments, receive an accounting of disclosures, request restrictions on use and disclosure, and request confidential communications — are exercised with the healthcare provider, practice, or pharmacy that treats you.

Please contact that provider directly and refer to its notice of privacy practices. If you are not sure who holds your records, the practice that scheduled or delivered your care is the right place to start.

If you believe your health information privacy rights have been violated, you may file a complaint with the provider involved and with the U.S. Department of Health and Human Services Office for Civil Rights:

You will not be penalized or retaliated against for filing a complaint.

Where we handle protected health information as a business associate under a services agreement with a healthcare organization, we do so on that organization’s behalf and under the terms of that agreement and the applicable business associate agreement. Requests about that information should go to the organization that treats you, which will direct them to us if needed.

4. Non-Discrimination

Blue Rock Physicians Group complies with applicable federal and state civil rights laws and does not discriminate on the basis of race, color, national origin, age, disability, sex, sexual orientation, gender identity, religion, veteran status, or any other characteristic protected by law. This applies to how we deal with the people and organizations we work with, to the providers in our network, and to our employees and applicants.

5. Accessibility
